Client Notes

 

This page guides you through the creation and management of client notes.

 

Introduction

Client notes are available to the organisation to document interactions with or activities related with the client. Examples of client notes can include the documenting of:

  • the participants engagement within the organisation.

  • the clients progress in reaching milestones either in respect of goals or services provided.

  • the occurrence of events, requests, outingr or visits.

  • changes in the client care needs.

  • recording conversations between the participant, care staff or third parties.

  • periodic plan reviews.

When combined with progress notes that are prepared by a support workers at the conclusion of a client service or CoS session notes, they can be used as evidence of service delivery. When working with notes raised through feature you do have the option of attaching documents against the note profile. It is at the discretion of the organisation as to whether this feature is used.

The Client notes are located on the client record under the Client Notes panel. Notes that are created are listed within a data-grid.


Navigating the Client note page

 

The following is an outline of the fields that comprise the Client Note page:

  1. Client Profile: This is a drop down field that lists the name of client for which the note relates.

  2. Note Date: This field is used specify the date that the note was written. The date can be specified as free text or by using the calendar picker

  3. Notes: This field is used to specify a detailed narrative of a circumstance, event, activity, service delivery, domain of care, change of needs or communication.


Creating a Client note

To create a Client Ongoing Note record observe the following steps:

  1. Select the Clients>Clients sub-menu to open the Client List page into the browser.

  2. Search for the Client record from within the Client list.

  3. Highlight the Client record to be edited.

  4. Either click on the edit button or double click on the Client record to open it within the Client form.

  5. Using the vertical scroll bar, slide the client page until you reach the Client Notes panel.

  6. Select the Add New Record icon to open the Client Ongoing Note record page within the browser

     

  7. Within the Note Date field input the date of the note.

  8. Within the Notes field input a detailed narrative related with an action or event.

  9. Select the SAVE button to create the note record.

  10. Select the CLOSE button to exit back to the client record.

 


Deleting a Client note

Although not recommended, and you are referred to the notes on record deactivation, to delete a Client Ongoing Note record observe the following steps:

  1. Select the Clients>Clients sub-menu to open the Client List page into the browser.

  2. Search for the Client record from within the Client list.

  3. Highlight the Client record to be edited.

  4. Either click on the edit button or double click on the Client record to open it within the Client form.

  5. Using the vertical scroll bar, slide the client page until you reach the Client Notes panel.

  6. Select the DELETE icon located to the right of the record that is to be removed. A pop-up screen will be presented.

     

  7. Select OK to confirm the deletion.

     

 

 


Activate and Deactivate a Client Note

To edit the activity status of a Client Ongoing Note record observe the following steps:

  1. Select the Clients>Clients sub-menu to open the Client List page into the browser.

  2. Search for the Client record from within the Client list.

  3. Highlight the Client record to be edited.

  4. Either click on the edit button or double click on the Client record to open it within the Client form.

  5. Using the vertical scroll bar, slide the client page until you reach the Client Notes panel.

  6. From the Client Notes data-grid, highlight and double click on the note record to open it within the Edit Client Ongoing Note record page.

  7. To deactivate the Client note record select the Deactivate button, upon screen refresh the input fields will be disabled. The Deactivated note will be retained within the data-grid.

  8. To activate the Client note record select the Activate button.

  9. Once done, you can exit back to the Client profile.
