Client Profile

 

Introduction

The Client Profile is a feature within the client record that allows the organisation to construct question based templates that allows for information to be collected across a variety of care and support domains that may be for example related with the learning, lifestyle, impairment, family, goals and aspirations, hobbies and interests, dislikes and triggers, medical or behaviors. Such information can then be used in the formulation of care plans, plan reviews and goal setting.

 

The Client profile is accessed through the client record by clicking on the About me menu which is located within the vertical menu bar


 

The following is an outline of the fields that comprise the Client Profile page:

  1. Client: This is a drop down field that lists the name of client for which the question will relate.

  2. Template: This is a drop down field that lists the category for which the questions will relate.

  3. Name: This field is used to specify the name of client.

  4. Description: This field is used to specify a detailed narrative of the profile record.

 

The following is an outline of the fields that comprise the Client Profile Details page:

  1. Name: This field can be used to specify the name of the question, input is data optional.

  2. Client Profile: This is a drop down field that lists the name of client for which the question will relate.

  3. Question: This is a drop down field that lists the question.

  4. Text: This field is used to specify an answer to the selected question.


Creating an About Me record

To create an About Me profile record observe the following steps:

  1. Click the About Me icon in the left vertical menu bar to open a record listing page within the browser.

  2. Select the New button to open the Client Profile page into the browser.

     

     

  3. Within the Template field, select from the drop down list the template that will be assigned to the client profile.

    If you find there is a need to include additional template values than these can be added by selecting :plus: icon located at the bottom right of the list to open the Client Profile Template details page, as shown below. Input the Template name, save and close the record. You will need to refresh the template listing

     

  4. Within the Name field input the name of the client.

  5. Within the Description field input a narrative as to the nature of the client profile record, this may include a reference to the questions that are being asked, which is helpful is multiple profile records have been created for the client.

  6. Click on the save button to create the client profile “About Me” record.

  7. Click the close button.


 Adding Profile Details

To create a profile detail record observe the following steps:

  1. Click the About Me icon in the left vertical menu bar to open a record listing page within the browser, or if your still on the client profile refer to step 3.

  2. Select the About Me record that is to be updated with profile details and click the Edit button to open the client profile within the browser.

  3. To add a new profile detail record select the new record icon to open a blank Client Profile Details page within the Browser.

     

     

  4. Inputting data into the Name field is optional.

  5. Within the Question field, select from the drop down list the Question that will be assigned to the client profile.

    If you find there is a need to include additional values than these can be added by selecting :plus: icon located at the bottom right of the list to open the Client Profile Template Questions details page, as shown below. On the Client Profile Template Questions details page select the appropriate values from the drop down lists within the Client Profile Template, Category and Type fields. Input the Question name. Save and close the record. You will need to refresh the question listing

     

  6. Within the text field input a narrative response to the question being asked.

  7. Click on the save button to create the client profile detail record.

  8. Click the close button.

  9. Repeat the steps 3 - 8 to assign additional questions.


Edit an About Me Record

To edit an About Me record observe the following steps

  1. Click the About Me icon in the left vertical menu bar to open a record listing page within the browser.

  2. Select the About Me record that is to be updated and click the Edit button to open the About Me profile within the browser.

  3. You can update the data listed within the Template, Name or Description fields.

  4. Once done, click on the save button within the ribbon bar to save the revised profile detail.

  5. Click on the close button to exit from the page.


Edit a Client Profile Detail Record

To edit the profile detail record observe the following steps

  1. Refer to steps 1 - 2 as outlined in Edit an About Me Record.

  2. Highlight the Profile detail record listed within the Profile Details data grid that is to be edited.

  3. Double click on the Profile record to open it within the Client Profile Detail page of the browser

  4. You can update the data listed within the Name , Question and Text fields.

  5. Once done, click on the save button within the ribbon bar to save the revised profile detail.

  6. Click on the close button to exit from the page.


Delete an About Me Record

To delete an About Me record observe the following steps:

  1. Click the About Me icon in the left vertical menu bar to open a record listing page within the browser.

  2. Select the About Me record that is to be deleted.

  3. Select the "Delete Button" in the upper right portion of the window.

  4. Select the OK button from the Delete Record dialogue, to remove the selected record. Deleting the About Me record will also remove Client Profile Detail records.


Delete a Client Profile Detail Record

To delete a Client Profile Detail record observe the following steps:

  1. Refer to steps 1 - 2 as outlined in Edit an About Me Record.

  2. Highlight the Profile detail record listed within the Profile Details data grid that is to be removed.

  3. Select the delete icon located to the far right column.

  4. Select the OK button from the Delete Record dialogue, to remove the selected record.


Activate and Deactivate an About Me Record

To edit the activity status of an About Me record observe the following steps:

  1. Click the About Me icon in the left vertical menu bar to open a record listing page within the browser.

  2. Search for the About Me record from within the list.

  3. Highlight the About Me record to be edited.

  4. Either click on the edit button or double click on the About Me record to open it within the Client Profile page of the browser

  5. To deactivate the About Me record select the Deactivate button.

  6. The About Me record page will refresh, all fields will be disabled. Please observe that once the record is deactivated you can not add Client Profile Details records.

  7. To activate the About Me record select the Activate button.

  8. Once done, you can exit back to the client
