Support Coordination Service Provider

 

Introduction

Service Provider records are a component of the Support Coordination record profile which list both the service requirement and the quotation for supports and service with service providers that will assist the participant in reaching their goals.


Navigating the Service Provider page

 

The following is an outline of the fields that comprise the Service Provider page:

  • Name: This field is used to specify the name of the support coordination service provider record, the input of data is optional, however data entered should be meaningful, as an example you could specify the provider name and support coordinator budget category.

  • Service Provider: This is a drop down field that lists the name of service providers that are registered within Brevity.

  • Quote Request Date: This field is used to input the date that the quote for the support/service was requested. The date can be specified as free text or by using the calendar picker.

  • Support Coordination Budget: This is a drop down field that lists the budget categories that have been registered against the support coordination record.

  • Support Coordination: This is a drop down field that lists the name of the support coordination record.

  • Met With Client: This field is comprised of two radio buttons to identify whether the client has been engaged with in relation to the Quote.

  • Comments: This field is used to input comments related with the quote or conversation undertaken with the client.

  • Quote Date: This field is used to input the date as listed on the quote. The date can be specified as free text or by using the calendar picker.

  • Quote Number: This field is used to list the providers reference number as presented on the quote.

  • Date From: This field is used to input the start date of the service as covered under the requested quote. The date can be specified as free text or by using the calendar picker.

  • Date To: This field is used to input the end date of the service as covered under the requested quote. The date can be specified as free text or by using the calendar picker.

  • Service Hours: This field is used to input the number of hours as covered under the requested quote.

  • Quoted Amount: This field is used to input the amount associated with the quote.

  • Budget Percentage: This field is used to reflect the quote amount as a percentage of the associated budget category.

  • Quote Accepted: This field is comprised of two radio buttons to identify whether the quote has been accepted.

  • Details of Provided Service: This field is used to input a detailed narrative of the quoted services being delivered to the client.


Creating a Support Coordination Service Provider record

Service Provider records can either be created at the same time that the Support Coordination record is created, or they can be created when the record is updated. To create a Service Provider observe the following steps which are based upon an update to the Support Coordination record:

 

  1. Select the Care Management>Support Coordination sub-menu to open the Support Coordination List page.

  2. Search for the Support Coordination record from within the Support Coordination List page.

  3. Highlight the Support Coordination record to be edited.

  4. Either click on the edit button or double click on the Support Coordination record to open it within the Support Coordination page.

  5. Select the ​ Add new record icon to open the New Support Coordination Service Provider record page within the browser.

  6. Within the Name field input a description for the Service Provider record.

    as an example you could specify the provider name and support coordinator budget category.

     

  7. Within the Service Provider field, either input the provider name or click the drop down list to select the required value.

     

  8. Within the Quote request date field specify the date that request for a quote on the required support or services was made. The date can be specified as free text or by using the calendar picker.

  9. Within the Support Coordination Budget field, either input the budget category or click the drop down list to select the required value.

     

  10. Update the Meet with client field value to YES if applicable.

  11. Within the comments field, if a there has been interaction with the client regarding the quote, input a narrative of conversation that occurred or particulars of the quotation.

  12. Within the Quote Date field specify the date of the Provider quote. The date can be specified as free text or by using the calendar picker.

  13. Within the Quote Number field input the unique reference number as provided by the provider.

  14. Within the Date from field specify the commencement date of the service or support. The date can be specified as free text or by using the calendar picker.

  15. Within the Date to field specify the cessation date of the service or support. The date can be specified as free text or by using the calendar picker.

  16. Within the Service hours field specify the duration of the service in full hours, if not known then assign 0.

  17. Within the Quoted Amount field specify the $value of the quote, if not known then assign 0.00.

  18. Within the Budget Percentage field specify the %value of the quote in relation the $value of the budget, if not known then assign 0.

  19. Update the Quotes Accepted field value to YES if applicable.

  20. Within the Details of Provided Service field, if the quote has been accepted, input a narrative of services and support being quoted and delivered.

  21. Select the Save button to create the record and update the Quotation data-grid with the details of the new record.

  22. Select the Close button to exit back to the Support Coordination page.


Edit a Support Coordination Service Provider record

To edit a Service record observe the following steps:

  1. Refer to steps 1 - 4 as outlined in the topic Creating a Support Coordination service provider record.

  2. Highlight the Service Provider record within the data-grid that is to updated.

     

  3. Double-click the Service Provider record to open Edit Support Coordination Service Provider record within the browser.

  4. Apply the required edits to the Service Provider record.

    Update the fields listed under the comments panel. Set the Met with Client field value to YES if the request for support and services was discussed with the client. Within the Comments field input a narrative of the discussions.


    Update the fields listed under the Details panel. Update the Quote Date field with the date of the quotation. Update the Quote number field with the supplier reference.

    Update with revisions, any of the applicable fields listed under the Service Details and Acceptance panels.

     

  5. Select the Save button to update the changes made.

  6. Select the Close button to exit back to the Support Coordination page.


Delete a Support Coordination Service record

Although not recommended, and you are referred to the notes on record deactivation, to delete a Service Provider record observe the following steps, off which there are two approaches available.

Deleting from the Service data-grid

  1. Refer to steps 1 - 4 as outlined in the topic above; Creating a Support Coordination service record.

  2. Highlight the Service Provider record listed within the Service data-grid that is to be deleted.

  3. Select the Delete icon located to the right of the record to present a pop-up screen

      

  4. Select OK to confirm the deletion. The Budgets data-grid will automatically refresh.

 

Deleting from the Service record

  1. Refer to steps 1 - 5 as listed above.

  2. From the Service data-grid, highlight and double click on the budget record to open the Edit Support Coordination Service record page within the browser.

  3. Select the DELETE button from the ribbon bar

  4. Select OK to confirm the deletion.

  5. Select the CLOSE button to exit the page

 


Activate and Deactivate a Support Coordination Service Provider record

To edit the activity status of an existing Service provider record observe the following steps:

  1. Select the Care Management>Support Coordination sub-menu to open the Support Coordination List page.

  2. Search for the Support Coordination record from within the Support Coordination List page.

  3. Highlight the Support Coordination record to be edited.

  4. Either click on the edit button or double click on the Support Coordination record to open it within the Support Coordination page within the browser.

  5. Highlight and double click on the service provider record to open it within the Edit Support Coordination Service Provider record page within the browser.

  6. To deactivate the Service Provider Record select the Deactivate button. Please note that the record will remain listed within the data-grid.

     

  7. To activate the Service Provider Record select the Activate button.

  8. Once done, you can exit back to the Support Coordination List.
