Introduction

Client information within the Brevity Self-Service Portal is reported through the client listing and client detail page(s).


Client Listing

This page is accessed by selecting the client menu and lists all the clients that have been assigned to a nominated staff member that is logged into the Brevity Self Service Portal. The listing of clients can be filtered either by the service start date, client activity status or a keyword search.

Clicking on the client name opens the client details screen to present an expanded information profile.

 


Client Details

 

Client details screen presents the following elements of information applicable with the client;

  1. Photo of the client , if supplied.

  2. Service Address and electronic contact details. The information contained within the panel includes the Clients name, gender, NDIS No: their preferred name, if there is a key worker nominated to provide support to the client, the client address, home phone, mobile number, email address, whether they have a requirement for SMS alerts and any associated tasks applicable with the clients support.

  3. Client goals. The information contained within the panel provides a diagrammatic view of the clients goals. In addition to viewing and updating existing goals new ones can be created.

  4. Client diagnosis. The information contained within this panel provides the clients primary diagnosis as listed against their profile.

  5. Nominated contacts. The information contained within this panel lists the contact persons registered against the client profile. Information includes the contact name, relationship to the client, whether they are an emergency contact, phone and email contact details.

  6. Therapy team. The information contained within this panel lists the support workers and allied health professional involved in the delivery of supports and services to the client. This panel allows for a service frequency to be specified against the team member. External Providers. The information contained within this panel lists the service providers that have been assigned against the client profile. Information includes the provider name and services provided, website, phone and email contact details.

  7. Listing of documents. The documents listed within this panel will have either been uploaded through the Brevity Self-Service portal or onto the client profile through the Brevity Care web application. Files listed through this panels are subject to the appropriate permissions.

  8. Service summary. The charts listed within this panel reflect the client funding broken down by period of time and funding remaining until end of the NDIS service agreement.

  9. Remaining/Available funding. The tables listed within this panel reflect the funding used and remaining by service type per funding profile.

  10. Support Budgets. This panel lists the budget records raised in relation to support coordination.

  11. Quotes. This panel lists quotes for a participant support or service, the associated budget and schedule of service delivery.

  12. Clinical and non-clinical session recordings. This panel lists the clients service history. Sessions created can be admin or clinical in nature or reflect a cancellation.

  13. Risk notification summary. as denoted in red is taken from the information contained within the Risk Notification field on the client profile. General information. as denoted in blue is taken from the information contained within the general notes field on the client profile.
