Introduction
The Client Profile template allows the organisation to construct question based templates that allows for information to be collected across a variety of care and support domains that may be for example related with the learning, lifestyle, impairment, family, goals and aspirations, hobbies and interests, dislikes and triggers, medical or behaviors. Such information can then be used in the formulation of care plans, plan reviews and goal setting.

If your version of Brevity doesn’t have the Client Profile Template configured within the Administration menu, please reach out to the support team at support@brevity.com.au. You can alternatively establish the client templates and questions through the About Me section of the client profile.
When registering template questions it is important to note that these are not specific with the template but rather they are questions that can be used across a multiple of templates that you maintain within brevity.

Creating a Client Profile Template
To create a Client Profile Template observe the following steps:
Select the Administration>Client Profile Template sub-menu to open the Client Profile Template List page.
Click the button to open a blank Client Profile Template record page.
Within the name field input a description of the template that will identify it from within the Client About Me page.
Click the SAVE button to create the Client Profile Template record. This will enable the Questions data grid.
Click the button to open a blank Client Profile Template Question record.
From the category field select a value from the drop down list that will be used to categorise the question.
From the type field select from the drop down list the value TEXT.
Within the name field input a description of the question.
Click the SAVE button to create the Client Profile Template Question record.
Click the CLOSE button to exit the page and return to the Client Profile Template page.
Click the CLOSE button to exit the template and return to the Client Profile Template list page.
Editing a Client Profile Template
To edit a Client Profile Template record observe the following steps
Select the Administration>Client Profile Template sub-menu to open the Client Profile Template List page.
Within the search field input the particulars of the Client Profile template.
Highlight the profile record that is to be edited and either click on the edit button or double-click on the record. Edits undertaken against the record include:

Adjustment to the Client Profile Template name.
✴Inclusion or the removal of client template profile questions. To assign additional questions select the
Add record icon button to register a new question through the New Template Question record page. To remove a question select the
Delete Record icon located to the right of the question.
✴Adjustment to the Client Template Question name. Double click the question name listed within the datagrid to open the Edit Template Question record page, apply your revisions, save the record and click CLOSE.
Click the SAVE button to update the Client Profile Template page.
Click the CLOSE button to exit the template and return to the Client Profile Template list page.
Deleting a Client Profile Template
To delete a Profile Template record, observe the following steps:
Select the Administration>Client Profile Template sub-menu to open the Client Profile Template List page.
Within the search field input the particulars of the Client Profile template.
Highlight the Client Profile Template record to be deleted.
Click the Delete Button in the ribbon bar.
Select the OK button to confirm the deletion of the selected record. Record listing will refresh..
Activate and Deactivate a Client Profile Template
To edit the activity status of a Profile Template record observe the following steps:
Refer to steps 1-2 under the topic Edit a Client Profile template Record.
To deactivate the profile record select the Deactivate button . Deactivating the record will set it to read-only.
To activate the profile record select the Activate button.
Click the CLOSE button to exit the page and return to the Client Profile Template list page.
