Navigating the Plan Review form

This page provides an overview of the Plan Review form.

The following is an outline of the fields that comprise the Plan Review page:

  • Name: This field is used to input a description of the clients plan review.

  • Client: This is a drop down field that lists the name of client for which the plan review relates.

  • Due Date: This field is used to define the last day that the client plan review must be concluded by. The date can be specified as free text or by using the calendar picker.

  • Unmet Need: This field is used to input a detailed narrative of any unmet needs identified by the organisation or raised by the participant which may not be being met or may be preventing goals from being achieved. Unmet needs may be financial , health, emotional or behavioral, having a lack of or access to the local community, transport, leisure and recreational, employment opportunities or not receiving the appropriate supports and services for their disability.

  • What hasn’t worked: This field is used to input a detailed narrative of the goals within the care plan as identified by the organisation or raised by the participant that have not been successful. The notes input may relate with any unmet needs.

  • What has worked: This field is used to input a detailed narrative of the goals within the care plan as identified by the organisation or raised by the participant that have been successful.

  • Additional Comments: This field is used to input in details that are not covered by the narratives input into the Unmet Need and What has and hasn’t worked fields. This input may relate to recommendations and actions that address issues related with unmet needs or aspects of the plan that have not been successful.

  • Periods: This record grid lists the established plan review periods.

  • Goals: This row lists the goal name and the individual goal tasks.

  • Goal name: This column panel lists an alternate name for the goal

  • Comments: This field on the column listing the goal task, is used to specify a narrative on goal outcomes.

  • Outcome: This drop down field on the column listing the goal task, is used to specify an outcome status.

  • Services: This label lists the services covered under the participants funding profile.

  • Current Situation: This field is used to specify a narrative on the nature of supports being provided for the funded service. These comments would reflect on service achievements, through progress comments & gaps.

  • Recommendation: This field is used to specify a narrative on any recommendations that can improve the supports being provided for the funded service,

  • Month: This field is used to set the month as a numeric value.

  • Date: This field is used to define the date that the plan review must be completed by. The date can be specified as free text or by using the calendar picker.

  • Plan Review: This field links the period to the plan review record.
