The Client Profile template allows the organisation to construct question based templates that allows for information to be collected across a variety of care and support domains that may be for example related with the learning, lifestyle, impairment, family, goals and aspirations, hobbies and interests, dislikes and triggers, medical or behaviors. Such information can then be used in the formulation of care plans, plan reviews and goal setting.

Miscellaneous Information

When configuring your templates:

  • You can refer to Client Profile topic for additional information.

  • If your version of Brevity doesn’t have the Client Profile Template configured within the Administration menu, please reach out to the support team at support@brevity.com.au. You can alternatively establish the client templates and questions through the About Me section of the client profile.

  • When registering template questions it is important to note that these are not specific with the template but rather they are questions that can be used across a multiple of templates that you maintain within brevity.

Navigating the Client Profile Template

Name: This field is used to input the name of the Template. The name of the template will be identified within the template field of the Client About Me form.

Add New Record: This icon is used to call the the Question form.

Question List: Data grid listing the template questions that were created through the Question form.

Delete Question: This icon is used to delete the question from the template.

Save: This button is used to create as well as save updates to an existing record.

Close: This button will close the Template Question page.

Deactivate: This button will deactivate the Template question.

Client Profile Template: This field lists the template record to which the question record relates.

Category: This is a drop down field that provides a listing of values that can be used to categorise the question. The options include Family, Learning, Interests and General. speak with brevity if you require additional categories included.

Type: This is a drop down field that provides a listing of values that allows to to define the characteristic of the field. At present text is the only field type that can be used.

Name: This field is used to specify the name of the question.

Creating a Client Profile Template

To create a Client Profile Template observe the following steps:

  1. Select the Administration>Client Profile Template sub-menu to open the Client Profile Template List page.

  2. Click the (plus) button to open a blank Client Profile Template record page.

  3. Within the name field input a description of the template that will identify it from within the Client About Me page.

  4. Click the SAVE button to create the Client Profile Template record. This will enable the Questions data grid.

  5. Click the (plus) button to open a blank Client Profile Template Question record.

  6. From the category field select a value from the drop down list that will be used to categorise the question.

  7. From the type field select from the drop down list the value TEXT.

  8. Within the name field input a description of the question.

  9. Click the SAVE button to create the Client Profile Template Question record.

  10. Click the CLOSE button to exit the page and return to the Client Profile Template page.

  11. Click the CLOSE button to exit the template and return to the Client Profile Template list page.

To edit the activity status of a Profile Template record observe the following steps:

  1. Select the Administration>Client Profile Template sub-menu to open the Client Profile Template List page.

  2. Within the search field input the particulars of the Client Profile template.

Deleting a Client Profile Template

To delete a Profile Template record, observe the following steps:

  1. Select the Administration>Client Profile Template sub-menu to open the Client Profile Template List page.

  2. Within the search field input the particulars of the Client Profile template.

  3. Highlight the Client Profile Template record to be deleted.

  4. Click the Delete Button in the ribbon bar.

  5. Select the OK button to confirm the deletion of the selected record. Record listing will refresh..

Activate and Deactivate a Client Profile Template

To edit the activity status of a Profile Template record observe the following steps:
